Abstract
The intrauterine fetal death(IUFD) is the death of the fetus prior to complete expulsion or extraction from it's mother when the fetus is over 20 weeks gestation or weighs more than 500 gm.
Improvement in antenatal and intrapartum care have reduced perinatal mortality, but the rapid advances in intensive neonatal care have had a dramatic effect in reducing neonatal deaths so that stillbirths have become relatively more important.
Therefore this study analysed 239 cases of IUFD among 8606 deliverires at Severance Hospital, Yonsei University during 4 years from January 1990 to December, 1993.
@ES The results of this study were as follow:
@EN 1. The incidence of IUFD was 2.78%.
2. The age distribution of mothers with IUFD was 18~45 years and highest in the 21~25 years(40.0%).
3. The parity of mothers with IUFD was the most highest in the nulliparous group (32.7%) and the next group was para-one(25.6%).
4. There were 16 cases(6.6%) with the previous history of IUFD and 61 cases(25.6%) with the previous history of the spontaneous abortion.
5. The most common gestational weeks when the IUFD was detected, was 37-40 weeks927.2%) and the most common presentation was cephalic(64.8%). The sex ratio of male versus female fetus was 1.09:1, and the cases of IUFD weighing less than 2500gm
were 192
cases(80.3%).
6. The hematologic findings of IUFD showed positive F.D.DP.(12.6%), a lower serum fibrinogen less than 150mg/dl(5.1%).
7. The mode of the delivery for IUFD showed that the induction of labor was most common(50.1%), and the spontaneous delivery 40.9%, the laparotomy was 14.1%.
8. The etiologic factors of the IUFD was unexplained causes(40.7%), preeclampsia (18.1%), cord problem(13.7%), congenital anomaly(8.8%), placenta abruptio(5.1%), and so on forth.
9. There were 72 cases(30.1%) of the maternal complication.
The most common complication was hemorrhage(46.5%), and the others were postpartum fever (21.0%), cervical laceration (14.0%), uterine rupture (13.5%) and so on forth.
As the results, we need carly detection of IUFD to minimize the complication of IUFD and perform the autopsy to elucidate the causes of IUFD.
